Bado classification of Monteggia fracture-dislocations

The Bado classification is one of the more widely used classifications for Monteggia fracture-dislocations and mainly focuses on the radial component. Four types are recognized and are generally based on the principle that the direction in which the apex of the ulnar fracture points is the same direction as the radial head dislocation 1-4.

Classification

  • I: anterior dislocation of the radial head
    • classic Monteggia fracture-dislocations
    • this type was originally described by Monteggia in 1814
    • most common type
  • II: posterior dislocation of the radial head
  • III: lateral or anterolateral dislocation of the radial head
  • IV: anterior radial head dislocation as well as proximal third ulnar and radial shaft fractures
