The most commonly adopted staging staging system for ovarian cancer is the FIGO staging system.

It is as follows:

  • stage I : tumour limited to the ovaries
    • stage Ia : tumour limited to one ovary, no malignant ascites / negative/negative peritoneal washings
    • stage Ib : tumour limited to both ovaries, no malignant ascites / negative/negative peritoneal washings
    • stage Ic : stage Ia or Ib with malignant ascites / positive/positive peritoneal washings
  • stage II : tumour involves one or both ovaries with pelvic extension
    • stage IIa : extension or implants on the uterus or fallopian tubes, no malignant ascites / negative/negative peritoneal washings
    • stage IIb : extension to other pelvic tissues, no malignant ascites / negative/negative peritoneal washings
    • stage IIc : stage IIa or IIb with malignant ascites / positive/positive peritoneal washings
  • stage III : tumour involves one or both ovaries with peritoneal implants outside the pelvis or with retroperitoneal lymph node metastasis
    • stage IIIa: microscopic peritoneal metastasis beyond the pelvis
    • stage IIIb: macroscopic peritoneal metastasis beyond the pelvis 2 cm or less
    • stage IIIc: : peritoneal peritoneal metastasis beyond the pelvis more than 2 cm or regional lymph node metastasis
  • stage IV : distant metastases including liver parenchyma
