Periapical cemento-osseous dysplasia, or periapical cemental dysplasia, is a subtype of cemento-osseous dysplasia.
There may be an increased female predilection.
It is represented as a benign lesion arising from undifferentiated cells of the periodontal ligament tissues.
Location
It occurs most commonly in the anterior mandible.

A wide spectrum of radiographic appearances has been described in previous reports.
- early-stage lesions: commonly present as periapical radiolucent defects that subsequently developed minute radiopacities
- later stage lesions: minute radiopacities tend to enlarge, coalesce, and undergo further substantial radiopacification; the radiopacities could consist of dense, cementum-like structures and/or areas with a ground-glass appearance with surrounding radiolucent peripheries 3
