Stage Description
T1 Clinically inapparent tumor, and not visible on imaging
T2 Confined to gland
T3a Extension through capsule
T3b Extension into seminal vesicles
T4 Direct extension beyond 3b
N1 Regional LN involvement.
M0 no mets
M1 mets

Gleason grading

Histological grading. A score of 1 to 5 is assigned to each of the two largest areas of tumor involvement in the samples obtained, based on the worst feature.

  • 1 = aggressive.
  • 5 = most aggressive.

These two scores are then added together to a Gleason score of 2 - 10.

  • 2 - 4 = low grade
  • 5 - 8 = intermediate grade
  • 9 - 10 = high grade

As this is obtained on biopsy specimens and correlates to prognosis, it enables treatement to be tailored.
