Testicular fracture

Last revised by Sonam Vadera on 16 Apr 2023

Testicular fracture is a break in the parenchyma of the testis as a result of blunt trauma.

Radiographic features

Ultrasound

A fracture line can be seen as a hypoechoic and avascular area within the testis but is only seen in 17% of cases 1.

Disruption of the tunica albuginea suggests testicular rupture, which requires surgical management.

Treatment and prognosis

The presence of blood flow demonstrates which region of the testis is still viable 2. Sometimes debridement along the line of the fracture is all that is required, while the rest of the testis is spared 2. Mild intratesticular fractures may be managed conservatively 3.
