Imaging of the left hip demonstrates a well defined sclerotic lesion of the neck of the femur. This lesion demonstrates a narrow zone of transition.
CT imaging corresponds with the plain film imaging, and demonstrates the presence of a lucent nidus, at the center of the lesion, constituting approximately 1/3rd of the lesion's volume.
No associated pathological fracture.
In the blood pool study there is abnormal early uptake medially in the left femoral neck. In the delayed bone scan, there is increased uptake medially in the distal femoral neck and slightly reduced uptake generally in the left hip.
MR images confirm a lesion is centered along the anterior medial femoral neck cortex. The centrally radiolucent region seen on previous CT corresponds to high T2 signal with enhancement post contrast. No cartilage signal or central calcifications seen within the lesion. No adjacent soft tissue mass or abnormal enhancement seen. No periosteal reaction seen.
Case Discussion
The appearances of the left femoral neck are consistent with the presence of a non aggressive lesion, most likely a small osteoblastoma.
