Sarcoidosis with hepatosplenic involvement

Diagnosis almost certain

Presentation

Patient was found to have an abnormal chest x-ray, performed because of long-standing respiratory problems.

Multiple interstitial nodules are seen throughout the lungs, so-called miliary image. Also note the multiple lymph nodes in the mediastinum and hili. Some lymph nodes have calcifications. Stage II disease. Also see the CT chest and abdomen for the mediastinal window settings.

Hepatomegaly with multiple small hypodense nodular lesions spread throughout the liver parenchyma. Also the spleen is involved showing somewhat larger hypodense nodules. Several small lymph nodes can be seen in the region of porta hepatis, celiac axis, superior mesenteric artery and gastrohepatic ligament.
