A large cystic area occupies the posterior fossa communicating directly with the fourth ventricle. The vermis is hypoplastic and rotated upwards. The aqueduct appears narrowed and there is prominent obstructive hydrocephalus. The overall size of the skull is increased.

MRI of an infant demonstrates features consistent with a Dandy-Walker malformation with associated aqueduct stenosis and obstructive hydrocephalus, with the triad 1. hypoplasia of the vermis with cephalad rotation, 2. cystic dilatation of the fourth ventricle, 3. enlarged posterior fossa).
