Hypersensitivity pneumonitis

Multiple soft centrilobular nodules throughout the lungs. No evidence of fibrosis. No overt mediastinal nodal enlargement on these limited axial slices on lung windows only.

Typical appearance of hypersensitivity pneumonitis, classical for subacute disease.

Case Discussion

Patient who is a bird fancier presents with shortness of breath, with CT evidence of hypersensitivity pneumonitis, most likely subacute. It is thought to represent a chronic inflammatory response to avian antigens (usually inhaled proteins in the dust of bird feathers and droppings).

Imaging appearances tend to resolve when the initiating antigen is removed.
