What is the main finding?
High T1 and T2 signal material is floating in the lateral ventricles (frontal horns) as well as scattered through the subarachnoid space.
What incidental and longstanding finding is also present?
Old left middle cerebral artery territory infarct.
Lipid-containing (and therefore strongly T1 and T2 (FLAIR) hyperintense ) material is floating on CSF in the anterior horns of the lateral ventricles. Multiple small droplets of T1 hyperintense lipid material can be seen in the subarachnoid space of both hemispheres. The originating (ruptured) cyst difficult to identify but is most probably located in the right parasellar area. There is no convincing solid enhancing component, although fat saturated sequence would be of benefit.
