Presentation
Elderly patient with sudden onset of the left-sided hemiparesis.

Non-contrast CT brain demonstrates subtle grey-white matter blurring and slight positive mass effect in the right frontal lobe.
CT angiography demonstrates a large defect involving the superior division of the right MCA territory with reduced vascular enhancement.
CT perfusion demonstrates a large defect involving the superior division of the right MCA territory.
Non-contrast done 3 days later confirms a right MCA superior division infarction.
Case Discussion
Initial non contrast CT demonstrates no definite abnormality, although in retrospect there is subtle blurring of the grey-white matter junction and slight swelling.
CTA and CT perfusion performed right after the non-contrast CT demonstrate a large region of avascularity involving the superior division of the right MCA territory with matching perfusion defect.
Follow-up CT performed three days later confirms an established infarct in the same distribution.
