Dyspnea and orthopnea.

Advanced bullous emphysema with right-sided predominance. Note peripheral reduction of lung markings with thick subpleural fibrous bands, and the flattened diaphragm. The AP diameter of the chest is also increased.

Confluous bullae better appreciated on the representative axial CT slice.
