Presentation
Referred for diagnostic mammography. She complains of a 3 weeks history of left breast discomfort and feels a ropy linear subcutaneous density running obliquely laterally from the nipple towards the left axilla. The breast is tender on exam. The mammogram is unremarkable.

Case Discussion
In clinical practice this is an uncommon diagnosis. The patients can complain of pain but usually the abnormality is palpable and felt by the patient. The key is to add ultrasound regardless of the mammographic findings. A high clinical suspicion will make the diagnosis together with the Doppler ultrasound images.
