Presentation

Known history of advanced colorectal carcinoma presenting at the emergency department with a distended and painful abdomen.

Patient Data

Age: 80 years
Gender: Female

Selected axial CT image using lung window shows the presence of free intra-abdominal gas that fills up an umbilical hernia. 

Case Discussion

This patient had an intestinal perforation due to the advanced neoplastic disease. Unfortunately, he died one day after the presentation. 

Pneumoperitoneum is gas within the peritoneal cavity and often is the indication of a critical illness.
