Navicular avascular necrosis (Köhler disease)

Diagnosis certain

Presentation

Pain at both feet for a few months, denied any preceding trauma. Mild tenderness over anteromedial aspect of both feet. No increased warmth or swelling. ROM both ankle joints were normal. Walking with a painful gait, with weight bearing over lateral aspect of both feet.

Patient Data

Age: 5 years
Gender: Male

The navicular bone appear wafer-like (thinned) and fragmented, and also demonstrates patchy sclerosis. 

Case Discussion

The findings are compatible with avascular necrosis of navicular bone which is also called Köhler disease.
