Dyspnea and hypoxia post respiratory tract infection. Recent ex-smoker.

Impression of minor bibasal fibrosis with some confluence at the right base

There is moderately extensive subpleural fibrosis involving the lungs bilaterally, worst at the right lung base.  Minor mediastinal lymphadenopathy is consistent with chronic lung disease.  Lungs and pleural spaces are otherwise clear.  Moderate prominence of the pulmonary arteries and slight prominence of the right-sided cardiac chambers to suggest development of pulmonary hypertension.

Conclusion: Pulmonary fibrosis with probable development of secondary pulmonary hypertension.

PATHOLOGY

Right lung wedge biopsies and pleural biopsy:  End-stage pulmonary fibrosis with features consistent with usual interstitial pneumonia.
