Gender: Male

Bilateral globus pallidus symmetrical foci of hyperintense signal in T2 and FLAIR with slight peripheral hyperintense margins on T1WIs. No diffusion restriction.

Diagnosis: Focal areas of signal intensity (FASI)

Case Discussion

Differentials include

  • carbon monoxide poisoning
  • venous infarction
  • bilateral basal ganglionic glioma
  • primary CNS lymphoma
  • toxoplasmosis

This patient is a known case of neurofibromatosis type 1
