How to differentiate between infarction in acute and subacute stage?
In subacute infarction there is gyral edema which exerts positive mass effect, gyral enhancement and hemorrhagic transformation. GRE may show some hemosiderin deposition.
Right inferior temporal and occipital gyral swelling and altered signal in cortical and subcortical area exhibiting restricted diffusion in DWI, bright signal in FLAIR and T2 and slight low signal in T1. No mass effect or hemorrhagic transformation.
MRA reveals occluded P1 segment of right PCA.
Diagnosis: Right PCA territory acute cerebral infarction.
