CT shows hydrocephalus and a hyperdense mass with peripheral punctate calcification with a epicenter at the pineal gland.

mri

MRI demonstrates a heterogeneous mass with its epicenter on pineal gland with extension/invasion to thalamus,midbrain and vermis with contrast enhancement. Pathology confirmed the diagnosis of pineoblastoma.

Case Discussion

Pineoblastomas are tumors that are best thought of as primitive neuroectodermal tumors (PNET) located in the pineal region and thus they closely resemble (both on imaging and on histology) medulloblastomasretinoblastomas and supratentorial PNETs. They are highly aggressive and infiltrative as is seen in this case.
