Presentation

Severe pelvic pain.

Patient Data

Age: 16 years
Gender: Female

The left ovary is hugely enlarged measuring 13 X 6.5 cm, pushing the uterus anteriorly and to the right side. It shows echogenic echopattern. No evidence of color flow seen inside. It’s surrounded by clear mild amount of fluid collection.

The left ovary is very large measuring 13 X 6.5 cm, pushing the uterus anteriorly and to the right side. It is hyperintense signal in all signals with multiple small follicles inside. Axial STIR shows twisted ovarian pedicle (whirlpool sign). It is surrounded by clear mild amount of fluid collection.

Case Discussion

Features are consistent with left sided ovarian torsion, with striking enlargement of the ovary.
