Peritoneal carcinomatosis

Presentation

The patient underwent hysterectomy, with unilateral adnexectomy several decades ago. Significant weight loss in the past months. Upon examination there was a palpable mass in the suprapubic region.

Widespread peritoneal inhomogeneous soft tissue densities, and free abdominal fluid.

A large septated, cystic mass was seen in the pelvis, originating from the area of both adnexa.

In the magnified key image, perihepatic fluid collection is shown with irregularities on the liver surface indicating possible further metastatic deposits.

Case Discussion

Ovarian cancer is usually at an advanced stage when peritoneal carcinomatosis is diagnosed. Ultrasound usually sees a large ovarian mass, without any significant finding on the peritoneal surfaces. The condition is accompanied by free abdominal fluid, and metastatic deposits are commonly seen on the surface of the liver.
