Adrenal myelolipoma

Presentation

Lung mass found on chest x-ray.

Patient Data

Age: 60 years
Gender: Male

A well-circumscribed fatty mass is seen in the right adrenal gland on these contrast enhanced images. This is most consistent with a benign myelolipoma. The differential diagnosis would include a benign lipoma which is much less common.
The third image was obtained three months later showing a stable appearance to the mass.

Case Discussion

Adrenal myelolipomas are relatively rare benign adrenal masses containing fat and hematopoietic elements. Occasionally they occur elsewhere in the retroperitoneum.

Myelolipomas account for about 2.6% of primary adrenal masses and have a prevalence at autopsy of 0.06%. In contrast, lipomas account for only 0.7% of primary adrenal masses.
