Hepatoblastoma

- a large left hepatic liver lobe fairly defined heterogeneous mass is seen, it measures about 12 X 8.5 X 10.5 cm in its main axial and CC dimensions. The mass shows heterogeneously hypointense T1 and hyperintense T2 with heterogeneous enhancement. The lesion shows patchy T1 hyperintensity within, likely related to mucoid degeneration as well as small signal void foci in all pulse sequences that could be due to calcifications. The lesion is displacing the stomach to the left and displacing the pancreas posteroinferiorly as well as displacing the bowel loops posteriorly
- normal spleen, pancreas and both kidneys
- no enlarged para-aortic lymph nodes
- no ascites
- the scanned lung bases show no definite signs of congestive heart failure, nor definite pulmonary deposits
Case Discussion
Large left liver lobe hepatoblastoma. The differential includes a hemangioendothelioma - which is a hypervascular lesion with cardiomegaly and signs of congestive heart failure or cutaneous hemangioma, maybe multiple.
Alpha-fetoprotein is positive in hepatoblastoma (as in this case), but negative in hemangioendothelioma.
