Ureteritis cystica

Case contributed by Ian Bickle
Diagnosis almost certain

Presentation

Right flank pain and sepsis.

Patient Data

Age: 60 years
Gender: Female
Fluoroscopy

Single 'shot' image from the time of nephrostomy tube insertion.

Long segment of mucosal irregularity in the proximal right ureter with associated narrowing.

Although patent the ureter is narrowed with mild post-stenotic dilatation.

Case Discussion

This single image was acquired at the time of nephrostomy tube insertion for pyonephrosis.

The appearances are in keeping with ureteritis cystica.

This is often associated with an underlying chronic infection. In this instance, tuberculosis is strongly suspected although not proven on urine cytology to date. Ureteroscopy and biopsy to be performed in due course.
