Presentation
Trauma. Fall down 12 stairs.
Patient Data
Patchy nodular opacities within the upper and midzones of both lungs.
Thickening of the paratracheal stripe.
No rib fractures seen. No pneumothorax or effusion.
Impression - In the setting of trauma, pulmonary contusions and aspiration could be considered. However, these may also be incidental findings and raise the possibility of sarcoidosis.
Multiple upper lobe nodules, in a peribrochial distribution.
Paratracheal and bilateral lymphadenopathy.
No evidence of traumatic injury.
Findings are highly suggestive of pulmonary and mediastinal sarcoidosis.
Case Discussion
Endobrochial biopsy of one of the mediastinal nodes (station 7 node) was in keeping with a granuloma, confirming the diagnosis of sarcoidosis.
