Presentation
Fall while water skiing. Hit by motor boat. Rapidly extracated from water. GCS 13. Intubated at scene. Compound occipital fracture with depressed fragment.
Patient Data
Large depressed skull fracture with associated hemorrhagic contusion.
Large left sided depressed occipital skull fracture with underlying cerebellar contusion and mass effect. In addition, there is a left sided transverse sinus thrombosis that extends into the jugular vein.
Case Discussion
Direct head strike has resulted in a large depressed occipital skull fracture with associated cerebellar contusion and a dural venous sinus thrombosis in the transverse sinus that extends into the proximal jugular vein.
When skull fractures occur in the region of the dural sinuses it is paramount to ensure that there is not a sinus thrombosis.
