Grossly distended food-filled oesophagus (sitting behind the left atrium) and proximal stomach up to the gastric band and collapse more distally. The patient has lost considerable weight (note the absence of subcutaneous fat) and does not want the balloon on the band deflated as she is happy with the weight loss despite the epigastric discomfort!
