CT scan shows a mass in the upper right pulmonary lobe that invades the mediastinum. On the images extended to the superior abdome is possible to identify masses in both adrenal glands, bigger at the right one. 

Case Discussion

Adrenal metastases are the most common malignant lesions involving the adrenal gland. 

This case represents a pulmonary primary tumor with metastatic lesion in both adrenals.
