Nipple adenoma

Case contributed by Chris O'Donnell
Diagnosis almost certain

Presentation

Palpable non-ulcerated lump in the left nipple with a clear discharge

Patient Data

Age: 45 years
Gender: Female
mammography

Minor nipple swelling. No retroareolar mass or stromal distortion.

ultrasound

Small vascular mass in the left nipple.

mri

Vascular mass with mixed perfusion pattern including washout in the medial left nipple.

Case Discussion

An enhancing nipple mass has few differential diagnoses. It is not Paget disease of the nipple (i.e. a more diffuse malignant process rather a discrete mass as in this case). Nipple adenoma is a type of intraductal papilloma that arises within the lactiferous ducts of the nipple thus it is a florid form of papillomatosis.
