Presentation

Prior radiotherapy for floor of mouth SCC.

Patient Data

Age: 91
Gender: Female

The left hemimandible shows a mixed lytic destructive process involving the bone associated with a fracture in this region.

These appearances are consistent with osteonecrosis secondary to prior radiotherapy on this region in a patient with a previous SCC of the floor of mouth

Case Discussion

Osteoradionecrosis of the mandible is seen in patients with previous radiotherapy for head and neck cancers. The changes maybe complicated by infection or  a pathological fracture.

Bisphosphonate therapy is also associated with mandibular osteonecrosis. Mandibular osteomyelitis can give a similar radiological appearance.
