The appearance is suggestive of a posterior glenohumeral dislocation with a lightbulb sign on the frontal projection.

The acromioclavicular joint is well aligned and no fracture is evident.

Case Discussion

Posterior shoulder dislocation is far less common that anterior dislocation and classically seen in patients post electrocution or seizures.
