Central fracture-dislocation of the acetabulum

Comminuted fracture of the left acetabulum (roof, anterior and posterior columns) extending into the ilium. Marked protrusio of the femoral head. Lateral pelvic wall hematoma (arrow). Further fracture of the left inferior pubic ramus.
Case Discussion
This fracture is typical of a fall from height onto the leg with transmission of force through the acetabulum. It is a significant injury and requires reduction and major metallic fixation if the patient is to return to ambulation.
