Road traffic accident; pedestrian vs car

Patient Data

Age: 75 years
Gender: Female

Multiple intracranial hemorrhages:

  • left temporal parenchymal hemorrhage
  • left acute subdural hematoma is stable
  • bilateral traumatic subarachnoid hemorrhage
  • right temporal extra-axial collection
  • left tentorium cerebelli and anterior falx cerebri subdural hematomas
  • right temporal parenchymal hematomas with surrounding edema

Right ICP monitor is noted with the tip in the region of the right basal ganglia with surrounding hypodensity representing edema.

Pneumocephalus. Left-sided calvarial and basal skull fractures are not well appreciated on these thick slices. Left mastoid and middle ear opacification.

Case Discussion

This is a case of a severe traumatic brain injury with monitoring of intracranial pressure considered standard of care, although there is a lack of data supporting this.
