Hypoxic-ischemic brain injury

Presentation

Trauma with tension pneumothorax and 10 minute downtime before restoration of spontaneous circulation. Not waking up off sedation on Day 5.

Patient Data

Age: 50 years
Gender: Male
mri

Symmetric bilateral high T2 signal and diffusion restriction within the lentiform nucleus, caudate nucleus and regions of the cerebral cortex. The cortical involvement is most pronounced within the posterior cortex of the precentral gyrus (motor strip) and the occipital cortex (visual cortex). Appearance is in keeping with acute hypoxic-ischemic brain injury. 

Case Discussion

A striking example of hypoxic-ischemic brain injury with grey matter cytotoxic edema most severe within the basal ganglia, precentral gyrus (motor strip) and the occipital cortex.
