Selected images of MRI showing a mixed solid-cystic tumor involving the right cerebral peduncle and thalamus compressing the third ventricle and causing midline shift. The tumor shows mild high signal on T2 and iso signal on T1 when compared to the adjacent brain. Contrast enhancement is vivid involving the solid components and the cysts' wall. 

Case Discussion

Pilocytic astrocytomas are tumors that occur predominantly in children and usually in the posterior fossa (~60%). In general they typically arise from midline structures, such as in this case. 

The diagnosis of this case was confirmed by biopsy and referred on the MRI request.
