Pseudotumor cerebri

Case contributed by Frank Gaillard
Diagnosis almost certain

Presentation

6 weeks blurred vision and headaches.

Patient Data

Age: 30 Years
Gender: Female
mri

There is enlargement of the suprasellar cistern with remodeling of the sella and flattening of the pituitary tissue. Prominent optic nerve sheaths with flattening of the posterior globes. Ventricular size is considered to be within normal limits. The transverse sinuses appear to be of narrow caliber without definite stenosis identified. No abnormal enhancement.

Conclusion:

The findings of an "empty" sella, distension of the optic nerve sheath and narrow caliber transverse sinuses would be in keeping with a diagnosis of idiopathic intracranial hypertension.

Case Discussion

These are fairly typical features of idiopathic intracranial hypertension confirmed on elevated opening pressures on LP.
