Perforated appendicitis

Presentation

Abdominal pain, tender in the right iliac fossa. Febrile.

Patient Data

Age: 65 years
Gender: Female

The appendix is markedly thickened and there is free gas and phlegmon within the right iliac fossa. There is an adjacent collection extending to the right adnexa and pouch of Douglas. There is thickening of the adjacent sigmoid colon which is likely secondary.

There are several lesions within the uterus, probably representing fibroids.

This case represents an acute appendicitis with perforation and abscess formation.
