Opacity and blurring of the right heart border in the medial right lower zone with shift of the heart to the left. 

x-ray

Lateral demonstrates pectus excavatum. No focal consolidation, collapse or pleural effusion. 

Case Discussion

A pectus excavatum is a common cause of right middle lobe opacity / loss of right heart border silhouette and mimics consolidation/collapse of the right middle lobe. It is easily diagnosed on a lateral projection.
