Post-partum cardiomyopathy

Diagnosis almost certain

Presentation

Tachycardia, bibasal crepitations, hypotensive.

Patient Data

Age: 30 years
Gender: Female
x-ray

Cardiac silhouette is enlarged. No pleural effusion. No frank pulmonary edema but the lungs appear congested. 

Case Discussion

This patient was 5 months post-partum, and had experience three months of orthopnea, persistent dry cough, and palpitations. No previous medical history. 

Transthoracic echocardiography demonstrated a severely dilated left ventricle with global hypokinesis and poor coaptation of mitral valve with severe regurgitation.
