Adamantinomatous craniopharyngioma

Case contributed by Dario Giambelluca
Diagnosis probable

Presentation

Headache and nausea for 25 days. Female patient who had not achieved menarche at 18 years old.

Patient Data

Age: 18 years
Gender: Female

Non-contrast CT shows a sellar-suprasellar mass (maximum diameter 5,5 cm x 5 cm), predominantly cystic, multiloculated, partially solid, with calcification inside. This lesion causes compression of the midbrain and the third ventricle, resulting in expansion of both lateral ventricles, with periventricular hypodensity, due to transependymal edema.

Case Discussion

Features are consistent with an adamantinomatous craniopharyngioma:

  1. pediatric patient
  2. headaches
  3. hormonal imbalances: primary amenorrhea
  4. involving both the suprasellar and intrasellar spaces
  5. CT features: multiloculated cystic lesions; solid component and calcification are present.
