Small caliber of the distal rectum with proximal dilatation. Fasciculation of the aganglionic segment is seen.

Significantly dilated colons with collapsed rectum. The transitional zone can be appreciated in all axial, coronal and sagittal planes. The rectosigmoid ratio is completely obliterated.
Case Discussion
Commonly a short segment of aganglionic distal colon/rectum is responsible for the disease. A late presentation like this case can result in significant proximal dilatation.
