Radial scar

Case contributed by Arlene Mou
Diagnosis almost certain

Presentation

Routine screening mammogram.

Patient Data

Age: 50 years
Gender: Female

Screening mammogram

mammography

Standard mammographic views demonstrate a region of architectural distortion in the upper, outer right breast. Further workup with coned, compression magnification views of the region is recommended, including view in the lateral projection.

Workup Mammography

mammography

Coned, compression, magnification views in the MLO and CC projections and a true lateral view of the right breast, confirm the presence of an architectural distortion in the UOQ of the right breast. Targeted ultrasound of the UOQ of the right breast and US surveillance of the remainder of the right breast and axilla is recommended.

Diagnostic Ultrasound

ultrasound

Unifocal 10 mm hypoechoic, irregular mass with posterior shadowing in the 10 o'clock, 3 cm FN position of the right breast. This finding corresponds in size and location with the mammographic region of interest. Normal appearing right axillary lymph nodes.

Case Discussion

The patient proceeded to core biopsy, which was non-diagnostic and then onto excisional biopsy:

Core biopsy: Benign breast parenchyma, no evidence of atypia, in situ or invasive malignancy.

Excision biopsy right breast: Radial scar and features consistent with previous biopsy.
