Case contributed by Safwat Mohammad Almoghazy
Diagnosis certain

Presentation

Abdominal pain. Right iliac fossa mass detected on ultrasound. CT suggested for further evaluation.

Patient Data

Age: 60 years
Gender: Female
ct

A well-circumscribed tubular, elongated cystic lesion with a “chicken drumstick” appearance is seen in the right iliac fossa. This lesion is lying vertically and is continuous with the terminal end of the appendix. The inferior part of the lesion is in contact with the normal right ovary. Mural curvilinear interrupted calcification, suggestive of bowel origin is seen. No surrounding fat stranding or abnormal lymph nodes are seen.

Findings are in favor of an appendicular mucocele.

Case Discussion

CT can reveal appendicular pathology and suggest the presence of an appendicular mucocoele. However, the ability to differentiate malignant and benign mucoceles with CT has been rarely reported in the literature.

The patient was referred for surgical consultation and an appendectomy was completed.
