Diagnosis almost certain

Presentation

Thumb trauma 2 weeks prior. Plain radiograph shows no fracture. Referred for ulnar collateral ligament evaluation by ultrasound.

Patient Data

Age: 50 years
Gender: Female
ultrasound

There is loss of continuity of the ulnar collateral ligament of the thumb at its distal attachment site. Controlled stress view shows joint space widening. The ruptured ligament is deep to the adductor aponeurosis. There is no associated bony avulsion.

Comparison with asymptomatic side is done, which shows an intact ligament.

Ultrasound findings favor rupture of the ulnar collateral ligament of the thumb without a Stener lesion.
