This is not a technical issue. This is known as the “flip flop” effect that occurs in serous bone marrow atrophy where T1-weighted sequence appears as fat-suppressed one while the fat-suppressed fluid sensitive sequences appear to be poorly fat-suppressed.
Diffuse high signal intensity of the subcutaneous fat and the bone marrow of both lower limbs on STIR sequence with corresponding homogenous intermediate signal on T1 weighted sequence. Note the swollen left lower limb compared to the right one.
