MR brain FLAIR sequences show ill-defined hyperintensity in the midbrain and symmetric hyperintensities in the posteromedial thalami.

Diagnosis: Artery of Percheron infarct.

Artery of Percheron is a rare single common artery supplying the rostral mesencephalon and medial thalami.
