Globular enlargement of the uterus with focal thickening of the junctional zone seen as an ill-defined inhomogeneous hypoechoic area of the posterior corporeal region.

The MR sequences show a focal thickening of the junctional zone (endometrium-myometrium) seen as a homogeneous low signal intensity on T2 (24 mm in this case), containing few foci of high signal intensity, compressing the endometrial cavity. Both ovaries appear normal, containing follicles with minimal effusion in the pouch of Douglas.
Case Discussion
MRI is superior to ultrasound in differentiating focal adenomyosis from uterine fibromyoma.
