Findings: There is lower anus intersphincteric fistulas arising at 2 and 7 o'clock with Seton sutures within. There is a left ischiorectal fossa abscess surrounded by inflammatory changes extending superficially into the left buttock subcutaneous fat, measuring approximately 7.1 x 1.3 x 2.8 cm (L x AP x T) - Vol. ~ 13.5 ml. Inflammatory changes also involves the levator ani muscle on the left with a favoured phlegmon communication between the collection and the fistula at 3 o'clock. The anorectal junction and imaged distal rectum are unremarkable. No suspicious lymphadenopathy.
