Discussion:

This unusual pattern of pattern of injury can be seen in blunt high energy trauma to the head and neck, consistent with the given history.

With the position and degree of displacement on CT, there is concern the alar ligaments supporting the craniocervical junction have been disrupted (grade III injury).

Immobilization and further investigation with MRI was recommended.
