Presentation

A boy with ataxia and developmental delay

Patient Data

Age: 20 months
Gender: Male
mri

Evidence of vermis aplasia associated with enlarged posterior fossa and torcular-lambdoid inversion is seen. Cystic dilation of 4th ventricle is also noted. 

These findings are compatible with Dandy Walker malformation.

Case Discussion

This is a classic case of a Dandy-Walker malformation.

  • hypoplasia of the vermis and cephalad rotation of the vermian remnant

  • cystic dilatation of the fourth ventricle extending posteriorly 

  • enlarged posterior fossa with torcular-lambdoid inversion (torcular lying above the level of the lambdoid due to abnormally high tentorium
